Helping Appalachian Parents and Infants Pospartum Services

 Eligibility:: Based on a combination of things like, your baby's health at birth, postpartum depression, parenting and difficulty with basic needs such as housing and transportation.

A combination of 24 monthly home visits, office visits or telephone contacts that include:

  • Periodic screening for Depression, Family Stress, other health behaviors and referral to needed services.
  • Educational Curriculum covering topics including: Maternal Health, Infant/Child Health, Family/Relationships, and Goal Setting (education, employment and relationships).
Case Management Services to link to, and pay for, needed services including:
  • Counseling for Depression or Family Stress.
  • Medication evaluations and Medications for Depression or Smoking Cessation.
  • Smoking cessation programs.
  • Medical evaluations for identified risk factors.
  • Assistance with transportation to identified services.
  • Link to other community resources.
  • Link to other community resources.
    • Eligibility:Any woman who is a current Maternal Child Services client.

      Intitial Screening for:
    • Depression
    • Family Stress
    • Smoking
      Oral Health Services:
    • Dental Screening and Cleaning
    • Oral Health Education
    • A Philips Sonicare Toothbrush and Toothpaste


      • Case Management services to link to:
    • Counseling for Depression or Family Stress
    • Medication evaluation for Depression
    • Smoking Cessation Program.
    Screening at Postpartum for Postpartum Depression.
